CareChoice isn’t a single-city agency. We operate across Pennsylvania, Michigan, Texas, and Florida — with additional states in the pipeline. Each state has its own Medicaid system, its own waiver programs, its own MCOs, and its own bureaucratic quirks. What stays the same is what we do: we help families access the Medicaid-funded programs that pay a family member to provide home care at home.

Philadelphia, PA

Our largest market. Philadelphia accounts for more than half of CareChoice’s business. We work within Community HealthChoices’ Southeast Zone with all three MCOs— AmeriHealth Caritas, Keystone First CHC, and PA Health & Wellness. We serve every neighborhood in the city and the surrounding counties
(Bucks, Chester, Delaware, Montgomery).

Our specialty: Participant-Directed Services through Agency with Choice, helping families get a relative hired and paid as a caregiver.

Pittsburgh, PA

CareChoice serves Pittsburgh and Allegheny County through CHC’s Southwest Zone. We work with all three Southwest MCOs including UPMC CHC. Same PDS/AWC pathway, same paid family caregiving structure as Philadelphia.

Detroit, MI

Michigan’s Home Help Program is one of the most accessible paid family caregiving programs in the country — including the option for spouses to be paid
in certain situations. CareChoice helps Detroit-area families navigate Wayne County MDHHS, the Home Help assessment, and provider enrollment.

For complex needs, we also guide families through the MI Choice Waiver and self-determination option.

Dallas, TX

Texas’s Consumer Directed Services through STAR+PLUS allows families to hire a relative as apaid attendant — with no waitlist for STAR+PLUS HCBS. CareChoice helps Dallas families with MCO selection, CDS setup, FMSA enrollment, and the employer/attendant structure.

Why Multi-State Matters

For many families, the multi-state presence is directly relevant. A daughter in Philadelphia caring for her mother may also have a father-in-law in Detroit who needs home care. A son in Dallas maybe exploring options for his parents in Pittsburgh. Families with loved ones across state lines benefit from working with one organization that understands multiple state systems — rather than starting over with a new agency in each market.

Even for families whose needs are entirely local, CareChoice’s multi-state experience means we’ve seen how different systems solve the same problems. When a Pennsylvania process stalls, we draw on what we’ve learned from Michigan’s more streamlined model. When a Texas family hits a CDS roadblock, we apply troubleshooting techniques from our AWC experience in Pennsylvania.
