MI Choice Waiver Explained: Home Care Options for Detroit Families
05/14/2026
Michigan’s Home Help Program gets the most attention when families search for paid caregiving options, and for good reason — it’s the simplest path. But there’s a second program that many Detroit families don’t know about, and for people whose care needs are complex or whose situations call for a wider range of services, it may be the better fit. It’s called the MI Choice Waiver, and it opens doors that Home Help alone cannot.
What the MI Choice Waiver Is
MI Choice is a Medicaid Home and Community-Based Services waiver administered through Michigan’s Area Agencies on Aging. Like the Home Help Program, it exists to keep people out of nursing facilities by funding care at home. The difference is scope: MI Choice provides a significantly broader set of services and includes a self-determination option that gives participants more control over their care.
The MI Choice Waiver serves individuals who meet a nursing-facility level of care— meaning their daily needs are intensive enough that, without adequate supports at home, a nursing home would be the alternative. This is a higher threshold than the basic Home Help Program, which serves people who need help with daily activities but may not require nursing-facility-level support.
In exchange for that higher eligibility threshold, MI Choice participants get access to a more comprehensive care package.
Services Available Under MI Choice
MI Choice covers everything the Home Help Program covers — personal care, homemaker services, medication reminders, mobility assistance — and extends well beyond it.
Adult day services provide structured daytime programming that includes social activities, meals, supervision, and in some cases health monitoring. For families where the primary caregiver works during the day, adult day services can fill the gap.
Home-delivered meals ensure your loved one receives regular, nutritious meals even when a caregiver isn’t present for every mealtime.
Personal emergency response systems (PERS) provide a wearable alert button that connects your loved one to emergency assistance 24 hours a day. For individuals who are at risk of falls or medical emergencies, PERS provides a critical safety net.
Home modifications fund physical changes to the living environment — grab bars, ramp installation, accessible bathroom conversions, widened doorways — that make it safer and more practical for your loved one to remain at home.
Assistive technology covers devices and equipment that support independence, from specialized communication tools to mobility aids to adaptive equipment for daily tasks.
Respite care gives the primary caregiver periodic breaks by arranging substitute care for a defined period.
Community transition services help individuals moving from a nursing facility back to a home setting, covering one-time expenses like security deposits, utility hookups, and essential furnishings.
Nursing services provide periodic skilled nursing visits for medical needs that go beyond what a personal care aide or family caregiver can address — wound care, medication administration, health monitoring.
This breadth of services makes MI Choice a true alternative to nursing home care, not just a supplement. For participants with complex needs, the MI Choice care plan can address nearly every dimension of daily life.
Self-Determination: Directing Your Own Care
MI Choice includes a self-determination option that functions similarly to Participant-Directed Services in other states. Under self-determination, the participant takes control of their care —choosing who provides it, setting the schedule, and directing the tasks.
This is where paid family caregiving enters the picture. A participant who chooses self-determination can hire a family member as their caregiver. The family member is paid through the program, and a fiscal intermediary handles payroll, taxes, and compliance — similar to how Michigan’s Home Help Program pays family caregivers, but within the broader MI Choice service framework.
The self-determination option is particularly valuable for families who want both the comprehensive service package of MI Choice and the personal touch of a family caregiver. Your loved one gets adult day services, home modifications, and emergency response through MI Choice’s provider network, while their daily personal care comes from a trusted family member who knows them best.
The same family relationships that qualify under Home Help generally qualify under MI Choice’s self-determination model, including the possibility of spousal caregiving in circumstances where the care needs exceed normal marital expectations.
MI Choice vs. Home Help: When Does MI Choice Make More Sense?
For many Detroit families, the decision between Home Help and MI Choice comes down to the intensity and complexity of the care needed.
Home Help is often the right fit when your loved one needs help with daily activities but their overall situation is relatively stable. They need personal care, meal preparation, housekeeping, and companionship, and a family caregiver can handle the full scope of their needs. The process is simpler, the enrollment is faster, and the program is more accessible.
MI Choice is often the better fit when your loved one has complex medical needs alongside personal care needs. They might benefit from skilled nursing visits, adult day programming, or home modifications. They may be transitioning from a nursing facility and need transition support services. Or their care needs are intensive enough that a single family caregiver providing personal care isn’t sufficient — they need a coordinated package of services.
The good news is that your family doesn’t have to choose in isolation. The assessment process will help determine which program (or combination of programs) fits your loved one’s situation. In some cases, individuals receive both Home Help services and MI Choice services, depending on their needs and eligibility.
How to Access MI Choice in Detroit
MI Choice is administered through Michigan’s Area Agencies on Aging, which means the Detroit Area Agency on Aging (DAAA) is your primary point of contact. Reach DAAA at (313) 446-4444 to inquire about MI Choice eligibility and start the intake process.
The enrollment path follows these steps. Your loved one must have active Medicaid coverage — apply through MI Bridges at newmibridges.michigan.gov if not already enrolled. Contact DAAA to request an MI Choice assessment. A supports coordinator from DAAA (or the contracted assessment entity) conducts an in-home assessment to determine nursing-facility level of care. If eligible, a comprehensive care plan is developed that includes all authorized services. If your family wants to self-direct and hire a family member as the caregiver, indicate this during care planning. The family caregiver completes provider enrollment, including background checks and payroll setup through the fiscal intermediary. Services begin according to the care plan.
The timeline from initial inquiry to active services is typically longer than Home Help — plan for six to twelve weeks depending on assessment scheduling and provider enrollment processing. Starting the Medicaid application and initial inquiry simultaneously (rather than sequentially) can compress the timeline.
Waitlists and Availability
Unlike some waiver programs in other states, Michigan’s MI Choice Waiver has generally maintained availability without significant waitlists in the Wayne County area. However, capacity can fluctuate based on state funding and enrollment volume. If there’s a wait, DAAA can advise on the expected timeline and help your family access Home Help services in the interim.
CareChoice Helps Detroit Families Navigate MI Choice
The MI Choice Waiver offers Detroit families a powerful set of home care options — but the enrollment process involves more moving parts than the Home Help Program. CareChoice helps families understand whether MI Choice is the right fit, connect with DAAA for the assessment, and get the self-determination and family caregiver pieces in place.
If your loved one’s needs go beyond what basic personal care covers, MI Choice may be the program that makes staying home realistic and sustainable.
