Instead of waiting through the traditional state process, we now work directly with your Managed Care Organization (MCO). We are actively partnering with UnitedHealthcare Community Plan in Michigan to help families move through the process more efficiently.

2. We Make the Call Together

Our CareChoice team will join you on a call with your MCO so your case manager can hear directly from you about your care needs. This step helps speed up the process and ensures your voice is heard.

3. Your MCO Schedules the In-Home Assessment

Once a case manager is assigned through your UnitedHealthcare Community Plan, they will contact you to schedule an in-home assessment. This assessment helps determine the number of home care hours you may qualify for.
