Home Care in Cherry Hill, Camden, and Pennsauken: NJ Families with Philly Connections

The Delaware River separates New Jersey from Philadelphia, but it doesn’t separate families.  Thousands of South Jersey residents in Cherry Hill, Camden, Pennsauken, Collingswood, Haddonfield, and Voorhees have parents, grandparents, or other family members living in Philadelphia who need home care.

If that’s your family, here’s the short version:  CareChoice serves Philadelphia, and your NJ location doesn’t affect your loved one’s eligibility.

How It Works

Your parent’s home care is governed by Pennsylvania Medicaid — specifically Community HealthChoices in the Southeast Zone. Eligibility is based on your parent’s Pennsylvania residency, their income and assets, and their functional care needs. Your New Jersey address doesn’t factor into any of it.

The application process, the MCO selection, the assessment, and the care plan all happen on the Pennsylvania side. You coordinate, we execute.

The PDS Option for NJ Families

If a family member who lives in or near Philadelphia — a sibling who stayed in the city, a cousin in Germantown, an aunt in South Philly— is willing to be the caregiver,
Participant-Directed Services lets them be hired and paid through Medicaid. You manage the big-picture coordination from Cherry Hill; the local family member provides the daily care and earns a paycheck.

If no local family member is available, CareChoice’s agency-directed model provides consistent, trained aides on a reliable schedule.

Getting Started

Same process as any Philadelphia family — just initiated from across the river.  COMPASS for Medicaid → PCA for referrals → CHC → assessment → CareChoice.
